Reaching for a bottle of sugar-free iced tea feels like an easy win. No added sugar, fewer calories — what's not to like? The catch is that manufacturers often need something to replace the flavor and mouthfeel that sugar once provided.

That something is frequently a sodium-based additive, such as sodium bicarbonate, used to stabilize taste and color. For most healthy adults, the extra sodium in a single drink is negligible. But for someone whose kidneys already struggle to filter sodium efficiently, these small additions can accumulate meaningfully over the course of a day.

Registered dietitians note a simple red flag: drinks that taste unusually smooth, leave a lingering aftertaste, or look artificially vivid often rely on added salts or flavor enhancers rather than natural ingredients. The safest bet, nutrition experts consistently agree, remains plain water — with the U.S. National Academy of Medicine's general adequate-intake guidance landing around 2.7 to 3.7 liters of total daily fluids for most adults, though people with kidney or heart conditions should follow individualized medical advice rather than a blanket target.

Salt Substitutes: A Genuine Health Trade-Off

Low-sodium salt and "light" soy sauce are popular swaps for people trying to cut back on sodium to protect their heart and blood pressure. The mechanism is straightforward: manufacturers replace a portion of the sodium chloride with potassium chloride, which tastes similar but carries different physiological effects.

For the general population, this trade-off is often a net positive. Large clinical trials have found that potassium-enriched salt substitutes can meaningfully lower blood pressure and reduce cardiovascular events. That evidence was strong enough that the World Health Organization recommended the switch broadly in 2025.

But there is an important exception. People with chronic kidney disease are specifically excluded from that WHO recommendation, because higher potassium intake can raise blood potassium to dangerous levels in patients whose kidneys can no longer clear it efficiently. Medical case reports describe this playing out in real patients: a hospitalized kidney disease patient developed dangerously high blood potassium after consuming a potassium-enriched salt product, and clinical researchers have separately documented life-threatening high potassium levels in patients taking blood pressure medications that also affect potassium regulation, even when kidney function appeared normal.

Severely elevated potassium, known medically as hyperkalemia, can trigger dangerous heart rhythm disturbances and, in extreme cases, cardiac arrest. That's why nephrologists advise that anyone with reduced kidney function should avoid low-sodium salt and similar potassium-based substitutes unless their doctor has specifically cleared it.

The Phosphate Additives Hiding in Plain Sight

Sodium and potassium get most of the attention, but phosphorus is quietly becoming one of the more pressing concerns in kidney nutrition research. Healthy kidneys filter out excess phosphorus continuously. When kidney function declines, that mineral can build up in the blood, contributing to bone and cardiovascular complications associated with chronic kidney disease.

The complicating factor is that not all dietary phosphorus behaves the same way. Inorganic phosphate additives, used by food manufacturers to extend shelf life, improve texture, and prevent clumping, are far more efficiently absorbed by the body than the phosphorus found naturally in whole foods such as unprocessed meat, nuts, and beans. These additives show up in items many people wouldn't think twice about, including fast food, canned or bottled beverages, and processed cheese products.

A 2025 cross-sectional study of packaged foods sold by the top U.S. manufacturers found that phosphate additives are absorbed far more efficiently than naturally occurring dietary phosphorus, yet they aren't accounted for in standard nutrition databases despite contributing substantially to total phosphorus intake. That's a problem for consumers, because U.S. food manufacturers currently use more than 50 different types of phosphate additives without any requirement to disclose the amount on nutrition labels. The National Kidney Foundation has formally petitioned regulators to change that, arguing patients and families are otherwise left to scan ingredient lists for terms like "pyrophosphate" and hope they've caught everything.

Practical Ways to Cut Sodium Without Losing Flavor

Nutrition experts point to several everyday adjustments that reduce sodium intake without resorting to potassium-based substitutes:

  • Lean on aromatics. Scallions, ginger, garlic, chili, lemon juice, and white pepper can replace salty condiments like satay or chili bean paste while still delivering bold flavor.
  • Use naturally savory ingredients. Mushrooms, corn, tomatoes, and root vegetables bring natural sweetness and umami to soups and stews, cutting the need for added salt.
  • Choose freshly prepared food when eating out, and limit heavily fried, braised, or sweet-and-sour dishes, which tend to carry hidden sodium loads.
  • If you've overdone it on salt, potassium-rich dark leafy greens (for people without kidney restrictions) or a cup of burdock root tea in moderation may help the body process the excess.

The Bigger Picture: Read Past the Front-of-Package Claim

None of this means sugar-free drinks, low-sodium salt, or processed convenience foods are inherently dangerous for the general public. The lesson is narrower and more useful: labels like "sugar-free," "zero-calorie," and "low-sodium" describe only one dimension of a product's nutritional profile.

For most healthy adults, a practical approach is to make water the default beverage, favor whole or minimally processed foods, and treat heavily processed sauces and snacks as occasional additions rather than staples — checking the ingredient list rather than relying on the front-of-package marketing.

For people already living with impaired kidney function, that label-reading habit becomes essential rather than optional, since sodium, potassium, phosphorus, and total fluid intake often all require individualized management alongside a nephrologist or renal dietitian.

Sometimes, as the old advice goes, the healthiest choice really is the simplest one: less salt, less sugar, more vegetables, and more water.
