Why Daycare Kids Get Sick So Often—And Why That's Actually Good News
New research from scientists at UCL, Cambridge, and Cornell confirms what many parents already suspect: starting daycare brings a wave of illness. But the same study shows this rough first year builds an immune system that pays off for years to come.

A Rough First Year, By the Numbers
A typical one-year-old starting nursery will pick up roughly 12 to 15 respiratory infections, two stomach bugs, and one or two rashes in their first year of care, according to a new review published in Clinical Microbiology Reviews involving researchers from UCL, Cambridge, and Cornell.
That's not a sign that something is wrong with a child's health. The five authors—all parents of young children themselves—set out to document just how often kids get sick after starting nursery, and why.
"Our lived experience of parents seeing a high burden of infections after a child starts nursery is backed up by the evidence," said co-author Dr. Lucy van Dorp, an infectious disease genomics researcher at UCL's Genetics Institute.
The good news: it gets better. Infection rates fall noticeably with each passing year, and the near-monthly colds of the toddler years become a distant memory by the time kids reach primary school.
Why Toddlers Get Hit So Hard
Newborns carry some borrowed protection from their mothers in the form of antibodies passed on during pregnancy. That shield fades within the first year of life—right around the time many families start daycare.
Co-author Dr. Leo Swadling of UCL's Institute of Infection, Immunity & Transplantation explained that once maternal antibodies wane, infants become more vulnerable because their immune systems have never encountered these bugs before. Every runny nose, upset stomach, or rash is often a genuine first encounter with a virus.
Young children also make natural transmission easier. They put objects in their mouths, rarely cover a cough, and spend hours in close contact with other kids—ideal conditions for viruses to spread through a nursery room.
But the researchers were clear that this isn't a hygiene failure. The high infection burden traces back to immature immune systems and the sheer transmissibility of common childhood pathogens, not to poor practices at daycare centers. Dr. Swadling described nursery as a kind of "boot camp" for the immune system, building resilience that lasts into later childhood.
Get Sick Now, or Get Sick Later
Parents who delay daycare to protect a child from illness may only be postponing the inevitable, the research suggests.
Children who start nursery early do get sick more often between ages one and five than kids who stay home. But once school begins, the pattern flips entirely: children without prior daycare experience start getting sick more often than their previously enrolled peers.
In other words, early infections often function as a kind of investment. The researchers found that early exposure in group childcare appears to offer some protection during the early school years, most likely because children have already built up immunity to common bugs circulating in classrooms.
Separate data from the Cleveland Clinic supports the same pattern: kids in daycare fall sick more than children cared for at home up until around age three, when infection rates between the two groups even out—and infection rates can even tip the other way once school starts.
Immune Training and the "Hygiene Hypothesis"
The pattern researchers describe echoes a long-standing idea in immunology known as the hygiene hypothesis: early exposure to a wide range of microbes may help train the immune system, potentially lowering the risk of allergies and autoimmune conditions later on.
Children raised on farms, who encounter a broader mix of microbes early in life, tend to show lower rates of allergies and certain autoimmune conditions than the general population. Researchers see a parallel effect playing out in daycare settings, though they caution that the underlying biology involves many interacting factors and is far from fully mapped out.
When a Sick Child Needs a Doctor
Most childhood infections resolve on their own with rest, fluids, and time. But certain warning signs call for prompt medical attention, according to guidance from the American Academy of Pediatrics.
Parents should contact a doctor if a child:
- Looks very ill, unusually drowsy, or extremely fussy
- Has a stiff neck, severe headache, severe ear pain, or trouble breathing
- Develops an unexplained rash
- Has repeated vomiting or diarrhea
- Runs a fever lasting more than three days in children age 2 or older
For infants younger than three months, any fever of 100.4°F (38°C) or higher warrants prompt medical evaluation, since babies this young have immune systems too immature to reliably fight off infection alone.
Signs of dehydration also deserve close attention: fewer wet diapers, dark urine, dry mouth, or a lack of tears when crying. Fast or labored breathing, along with unusual sleepiness or difficulty waking, can signal that a child needs urgent care.
What Parents Can Actually Do
Frequent infections during the daycare years are, for most children, simply part of normal immune development rather than a red flag. Still, parents aren't powerless—several concrete steps can reduce both the frequency and severity of illness.
Vaccination remains the single most effective tool available. The study's authors stressed that keeping children up to date with all recommended vaccines—including newer combination shots covering measles, mumps, rubella, and chickenpox—is one of the most effective ways to protect against serious nursery-related illness.
Keeping sick children home. Corresponding author Dr. Charlotte Houldcroft of the University of Cambridge noted that following nursery guidelines on keeping unwell children at home—sometimes for a day or two after symptoms clear, depending on the illness—makes a real difference in limiting spread through the group.
Everyday habits. Regular handwashing with soap and water, time spent outdoors where transmission risk is lower, and adequate vitamin D intake all support a child's developing immune defenses.
Breastfeeding, where possible, can offer additional protection, since breast milk contains antibodies that help shield infants during this vulnerable window.
Timing matters. The first two to three months of life carry the highest risk from infection, making extra caution especially worthwhile during that narrow window. After that point, exposure through daycare and social contact becomes a normal and largely beneficial part of immune development.
The Outlook
Employers, too, have a role to play. Dr. van Dorp noted that workplaces need to recognize it's normal for parents of young children to regularly need time off to care for sick kids—a burden that eases as children grow older.
For families in the thick of the daycare years, the takeaway from this research is reassurance rather than alarm. The relentless cycle of colds, stomach bugs, and rashes is difficult to live through, but it is temporary—and it appears to leave children better equipped to handle the infections still ahead.
